Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Currently Simply Thrive Psychotherapy, PLLC takes the following insurances:
- Labor and Industries (LNI)
- Humana (Medicare)
- Tricare/Tricare for Life/ Champ VA/ VA Choice
- Blue Cross/Blue Shield
- Community HealthPlan
- We can also provide payment receipts for clients attempting to submit out of network benefits
Currently, cash, check and credit card are accepted for payment. Please ask about insurance benefit cards or other FHA plans during your intake phone call.
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 48 hours in advance, you will be required to pay a $50.00 fee for - this WILL apply for a no show to a scheduled appointment as well as a late cancel (cancellation after the 48 hours).
